Please complete and submit the following form to formally notify QM of your intent to subscribe.

The Institution Representative will receive a Quality Matters Program Subscription Agreement and Invoice. Subscription is not confirmed and services will not begin until signed contract is returned. Payment is due within 30 days upon receipt of invoice.

Institution Representative:

- Will be Quality Matters' liaison within the institution

- Should be the individual with day-to-day responsibility for implementing Quality Matters within the institution

- If desired, institution may add a separate contact for contract and billing matters

Please upload a file (.txt,.doc, or .pdf) listing each Member Institution to be included in this subscription. Include the following: Member Institution Name, Website, Institution Representative, Title, Address, Phone, Fax, Email, and Statewide System/Consortium.
